John Dailey Jr., 49

John was killed in August 2013 while working for Transmit PM in Coats, North Carolina. He fell 200 feet after his harness failed.

CDOL investigated the incident and found no violations.

---- On August 12, 2013 John fell 200 feet to his death that day in Coats, NC. While doing work for Sprint. He was employed by Transmit PM. According to OSHA, he had a malfunction with his safety harness when trying to hook off (tie off??) but investigation, to date, has not been closed. John had 2 kids, Brooke & Landon. He was definitely the comedian of our family. John had a heart of gold and never met a stranger. He was a certified climber, instructor & troubleshooter. He worked in some type of construction all his life. He'd been climbing towers over 15 years. He loved climbing towers and was not only dedicated to his work, but he took pride in it. He was a technology buff. I made a video of him and link is on my page. How do you sum up someone's life in less than 15 minutes? Emilo R. DeLeon, 19

Emilio was killed on the job in August 2010 while working at Scarborough Construction in Grand Island, Nebraska. He came in contact with high power line while in a bucket and was electrocuted.

Federal OSHA investigated the incident and proposed a $23,800.00 penalty for four serious violations. In an informal settlement the penalty was reduced to $16,660.00 for four serious violations. This case is closed.

Ken Dimon Jr., 55

Ken was killed on the job in September 2011 while working for Waste Management in Cocoa, Florida. He was killed when crushed between a car and a truck.

This case is not in OSHA's jurisdiction.

---- Ken was employed as a driver for Waste Management in Cocoa, Florida when he was killed while loading brush into the back of the truck. A car ran into the back of the truck wedging Ken between the car and truck, killing him instantly. The driver of the car was found guilty of speed, special circumstances and failure to use due care. She received a ticket for $169.00, a $1,000 fine and loss of license for six months. Ken leaves behind his loving wife, Kathleen, daughter Savannah, son Michael and his faithful dog Lucky.

Shon Disney, 32

Shon was killed on the job in September 2015 while working for Zito Media in York, Nebraska. He was killed after a rollover incident occurred after working multiple long shifts.

Jared Dodson, 25

Jared was killed on the job in January 2012 while working for Southwest Airlines Dulles International Airport in Dulles, Virginia. He was killed when he was hit by a “mobile lounge” passenger transport. Dodson died the next day at an area hospital from complications.

VOSH investigated the incident and no violations were issued. This case is closed.

---- It's been a long difficult road for us the past 2 1/2 years! This was a very preventable accident and never should have happened. Our hearts have been broken by the taking of our son's life. The only mistake our son made that day was going to work!.

Christopher Andrew Doughty, 18

Christopher was killed on the job in February 2007 while working for Intech in Antonio, Texas. He was killed when caught in auger.

Federal OSHA investigated the incident and proposed a $7,250.00 penalty. In an informant settlement the penalty was reduced to $3,500.00.  This case is closed.

Corneilius Johannes Du Plessis, 27

Corneilius was killed on the job in June 2014 while working for Doepke Farm in Washburn, North Dakota. He was killed when he was engulfed in grain.

Steve Durbin, 57

Steve was killed in September 2003 while working for Kokosing Construction Co. in Heath, Ohio. He died when a trench collapsed on him.

Federal OSHA investigated the incident and assessed a $15,000.00 penalty for three serious violations. In an informal settlement the penalty was reduced $7,500.00 for one serious violations and one other than serious violation.  This case is closed.
