Nicolas M. Van Dyke, 28  

Nicolas was killed in June 2011 while working at Lockhart's Services, Inc. in Elkhorn, Wisconsin. He died when a car fell off of an automotive lift (aka "hoist).

Federal OSHA investigated the incident and proposed a $4,200.00 for one serious violation. There were no reductions. Inspection: 77208.015 - Lockhart Service, Inc.

Jacob Von Kopfman, 21

Jacob was killed on the job in August 2011 while working for Hayden Tower Service Inc in Brookfield Missouri. He fell 84 feet while dismantling a tower.

Federal OSHA investigated the incident and proposed a $ 9,800.00 penalty for two serious violations. In an informal settlement the penalty and violations were deleted. The Case is closed.

Andreas H. Vionis, 35   

Andreas was killed in April 2006 while working for KMC Oiltools/Helmerich in Newton County, Texas. He was electrocuted.

Federal OSHA investigated the incident and proposed $0.00 penalties and no citations. Inspection: 308591965 - Kmc Oiltools, Inc.
