Keith Everett, 53

Keith was killed on the job in January 2014 while working for International Nutrition Plant, Nebraska. He was killed when he was crushed when the building collapsed.

David Ball, employee of International Nutrition was also killed in this incident.

Federal OSHA investigated the incident and proposed a $120,000.00 penalty for seven serious, one willful, one repeat and two other violation. In an informal settlement the penalty was reduced to $78,000.00 for four serious, two repeat and three other violation. This case is still open.
